Consent Forms
Spay/Neuter Consent Form
This consent form is modelled after the ASPCA Spay/Neuter Alliance's consent form. It has many of the key components that should legally be in a surgical consent form. Note: The Spanish version here has contains both English and Spanish. Our approach is to have one consent form that works for both English and Spanish speaking populations. If you have a large Spanish speaking population, you may consider this version.
Wellness Consent Form
This consent form is for wellness appointments of all types. This includes high volume vaccine clinics and other non-surgical wellness services. If you need a non-spay/neuter surgical wellness form, we suggest you adapt this one to include any type of wellness service.
Recheck Consent Form
This form can be used for rechecking patients.

Drug Baggie Insert
This form can be used to insert into your drug baggies to facilitate intra-team communication. It is for groups that are paperless.
High Risk Waiver
This form can be used as a high risk waiver.
Recovery Team Communication Tool
You can print this document and have it laminated. When a patient has an issue and you want to alert the recovery team, you can use a dry erase marker to mark off the issue, or write in a new one. This card stays with the animal all through recovery, and perhaps even until pick up.
Don't Confuse the Ace
For those using Acepromazine for pre-sedation you likely have a bottle of Dilute Ace and Full Strength Ace. You can print these handy card on two different colors of paper and sit the Dilute Ace on the Dilute Ace card and the Full Strength Ace on the Full Strength card. This is an extra safety precaution to help drug drawers not mix 'em up.

Checklist Points to Go Over With Owners
Sometimes it's helpful to be able to hand the checkout person a checklist of points to go over. We use these at First Nations Vet clinic for the 75 volunteers that help us each year. This ensure each owner hears the same information. It's a quick way to empower a volunteer, or new employee, to go over aftercare instructions. We laminate these on brightly colored pieces of paper.
